Building a Questionnaire or Survey within the FactFinder Solution powered by Jitsutech is super easy. I have been using this software for many months and am really enjoying the versatility of developing Health templates. Today, I wanted to share a Health Opinion Survey template.
Generally, healthcare surveys offer administrators insight in hospitals, colleges and healthcare organizations on topics such as quality, affordability, preferences and other related information. This specific health opinion survey was designed to identify psychoneurotic disorders for adults in rural communities. I have added a unique four-point answer scale which includes often, sometimes, seldom and never. Further, this template can also be used as an interview tool if your target audience has limited or no Internet connectivity.
