I have conducted over 100(+) Survey Research projects and I can tell you that adding a Consent Form to your Canadian Hosted Survey Software is important.

A Consent Form can be easily added to your Online Survey. Simply write out your consent form and have a colleague review it to ensure their are no spelling or grammatical errors. Then you can easily copy that into the Canadian Hosted Survey Software.

Here are some helpful tips to assist you in writing your Consent Form:

• Identify the purpose of the survey research;
• Discuss the time required to fill in the survey;
• Clarify how you will minimize risks;
• Describe the overall benefits of the survey research;
• State data privacy and confidentiality; And
• Finalize with your name and contact information.

Also, the The Panel of Research Ethics is a great resource for promoting ethical conduct where research involves human participants. In Chapter Three, of the Tri-Council Policy Statement, you will find a very clearly written Consent Process. It is actually a very good read.
